Information you submit through our chat assistant and appointment or reschedule request forms (name, contact details, requested dates, and the services you express interest in).
Standard website usage data collected by our hosting and accessibility providers.

How we use it

To respond to your appointment requests and provide care if you become a patient.
To process payments and memberships. Payments are processed by Square and financing by Cherry; we do not store your card details on this website. Supplement orders are fulfilled by WholeScripts and prescription peptides by our pharmacy partners.
We do not sell your personal information.

Protected health information (PHI)

Health information you provide in the course of care is protected health information under HIPAA and is used and disclosed only as permitted for treatment, payment, and health care operations, or as otherwise required by law. Urowell Integrative Care is a cash-based, direct-pay practice. Integrative wellness services are generally not covered by insurance. Pricing shown on this website is subject to change and will be confirmed before treatment.
All treatments and prescriptions require an in-person or telehealth clinical evaluation. Not all patients are candidates for every treatment, and individual results vary.
Membership fees are billed by Square according to the plan you select. Contact us to change or cancel your membership.
